Cream, lotion or ointment: which is best for dry skin

A gel is the lightest, then a lotion. A cream is thicker and holds more oil, which makes it more hydrating, and an ointment is the thickest of all, which the AAD calls the most effective for dry skin; it tells people with dry skin to start with a cream or ointment and skip lotions altogether, and says perimenopausal people and everyone 50 and older should use a cream. The Cleveland Clinic differs on the face: its skin is thinner, so it says a gel or lotion is generally better there unless the skin is very dry.

In practice: a cream morning and night; on cracked patches, a balm by day and an ointment on top at night. That is why this guide has a balm and an ointment as well as creams.

How a moisturizer fixes dry skin

Dry skin loses water faster than it holds it. A good moisturizer works three ways at once, as reviews of the research describe them (Purnamawati 2017):

  • Humectants draw water in: glycerin, hyaluronic acid, urea, sorbitol. Glycerin is among the best studied (Fluhr 2008).
  • Emollients soften and fill the cracks between flakes: squalane, shea butter, plant oils.
  • Occlusives slow water loss: petrolatum is the strongest, then mineral oil and dimethicone.

Barrier lipids are their own group. Ceramides, cholesterol and fatty acids are what the skin’s outer barrier is built from, and they repair it best together (Man 1996). Some guides file ceramides under occlusives; they are the barrier’s own lipids, not a seal on top of it.

“Fragrance-free” does not always mean free of scent

Fragrance is one of the things the AAD says to avoid on dry skin, and the Cleveland Clinic goes further: skip botanicals too, including lavender, peppermint and citrus oils, because a product labeled fragrance-free is not necessarily free of allergens. Two popular dry-skin creams show why the list matters more than the claim:

  • SkinCeuticals Triple Lipid Restore 2:4:2 is listed as fragrance-free by two of the big guides that rank for dry skin, but SkinCeuticals' own page names an essential-oil blend of lavender, rosemary and peppermint oil.

How to use a moisturizer on dry skin

  1. On damp skin, within three minutes of a shower or bath, as the Cleveland Clinic advises: it traps the water in.
  2. Several times a day, the AAD says: after bathing, after washing your hands, and whenever skin feels dry.
  3. Seal it at night on very dry patches with a thin layer of ointment, the last step (“slugging”). Skip this on acne-prone areas.
  4. Keep washing gentle: short, warm showers and a gentle cleanser, as the AAD advises, and no scrubbing flaky skin.

What to check on a moisturizer label for dry skin: six things

Cosmetic labels list ingredients by amount down to 1%, then in any order (21 CFR 701.3); our guide to reading an ingredient list explains the rest.

  • A cream or an ointment. The AAD tells people with dry skin to bypass lotions.
  • Something to hold water in. Petrolatum, dimethicone, mineral oil or shea butter: all on the AAD’s dry-skin list. Only 3% of face moisturizers in our catalog list petrolatum.
  • Something to draw water in. Glycerin or hyaluronic acid, both on the AAD’s dry-skin list, or urea or sorbitol, which the Cleveland Clinic also lists.
  • Barrier lipids. Ceramides with cholesterol and fatty acids work best together (Man 1996); 19% of face moisturizers in our catalog list a ceramide.
  • No fragrance and no essential oils. “Fragrance-free” on the front does not settle it: read the list for lavender, peppermint or citrus oils and allergens like limonene and linalool.
  • No alcohol denat. or AHA near the top. The AAD says to avoid alcohol and alpha-hydroxy acids when skin is dry.

Why is my skin still dry after moisturizing?
